Various Exam Enhancements

After collecting feedback from various users we have collated the following enhancements to the exams module:

• The exam module should optionally, notify examiners when:
  • An exam has been submitted by the examinee
  • An examinee has passed or failed if automatically marked
  • when an exam closes if there are unattempted or unsubmitted exams.
• The exam module should notify examinees when results become available.
• The option to group questions by question category when randomly selectin from the question bank.
• The option to specify a time limit for how long an examinee has to write an exam once it is opened. 
• The ability to randomise the question selection per examinee.
• The ability to automatically re-issue exams up to a set limit when an exam has failed (e.g. three attempts). This should have the option as per the point above to randomise questions on each re-issue.
• The ability to add exams to courses.
• When a recency is updated by passing an exam a link should be generated to that person's exam results summary, as check forms currently do.
• Add filters to exam results summary to allow filtering by exam date, exam category, exam status (archived or not).
• Add filters to personnel results summary for base, department,  and pass/fail/unattempted.
