Form Module Improvements - Update rules for Personnel Picker

Problem: Can’t auto-populate related data when personnel chosen.

Actionables:

• Allow “On select” rules: fill department, base, contact from personnel profile.

• Support clearing those fields if personnel is cleared.

Success criteria:

• Dependent fields auto-fill/clear correctly on selection changes.
