Assigning cause codes in a finding of a safety report

In a safety investigation (safety report - Findings/Action tab) - request the ability to assign a cause (preferably multiple causes) directly in the Findings/Action tab, and also in a Finding, rather than only in the 'action'.

Ideally, when raising an action, it would be good to be able to then identify which cause or causes the action seeks to address. (e.g. drop down list of identified causes the action relates to).

Discussion:

Currently cause codes can only be assigned to an Action.

During a safety investigation the aim is to find out why (causes) the occurrence happened, and prevention of future possible occurrences due to the same causes. The investigation flow is generally: make a finding, identify cause(s), and then propose/identify actions to reduce the possibility of the cause(s) occurring again.

Causes may be identified in the Forum (essentially a finding), or identified in a Finding; thus it would be beneficial to be able to assign cause(s) either directly (suggest in 'Findings/Actions' tab, add 'Add Cause' functionality next to the 'Add Finding') or as part of a Finding.

Currently the Air Maestro system only allows a cause to be assigned as part of raising an action, and while that might be the flow for an audit, it is different in a safety investigation.

Causes should be able to be also used over time to identify trends through statistical analysis. Having to raise an action to then assign a single cause and then repeat for a second or multiple other causes is extra work that shouldn't be need, especially as if the causes are known (forum or finding) and an action is not required. its only generated to capture the cause.

In summary request the ability to assign causes either:

a. On its own (suggest in 'Findings/Actions' tab, add 'Add Cause' functionality next to the existing 'Add Finding'); and/or

b. In the Finding (similar to the method used in 'Actions')

Also request the ability to assign multiple causes (not just one) in the above (including Actions).
