Add a Risk Assessment Matrix to the Custom Forms controls

A number of customers require the ability to insert Risk Assessment Matrices (RAMs) into custom forms.

The only way in which to achieve this, currently, is by creating drop down lists for the Likelihood and Severity fields, and then calculating the Risk Factor by means of a calculated field. This is extremely laborious, to say the least.

As a User, I would therefore like to have access to a Risk Assessment Matrix control which is available for ALL custom forms (i.e. Safety Custom tabs, Forms, Flight Records and the Custom Fields section of the Scheduler).

Additionally, the product of the RAM control (i.e. the Risk Factor) should be a value that can be referenced in a calculated field. For example, if I wish to calculate the trend between RAM controls used to derive an Initial Risk Factor and a Revised Risk Factor, I should be able to do so using an IF(THEN) formula.
