When raising an action the system prompts your to add the cause category (mandatory function). The problem is the cause category is unknown at the point of raising the action as there has been no investigation at this point.
After the investigation and at the point of closing the action, the cause category has already been set and maybe incorrect. There is no prompt to review the category and change it accordingly.
If the cause category is not changed (ie missed) at the point of closure, any reporting later on to determine typical cause categories, will be flawed and inaccurate.
It would make more sense if the cause category was a mandatory field at the point of closing an action.
