Adding extra risk identifiers in safety reports e.g. (When a likelihood and consequence are chosen)

It would be good for the risk codes (1A, 2A, 3A etc.) to appear as well as the risk index name e.g. (Very low, low, medium, high, very high). At the moment only the risk index name appears, not the code identifier as well.
